Advance Care Planning

This community presentation discusses what is "Advance Care Planning" and why is it important. How to think about the type of medical care you want in the future and how to discuss this with the people that need to know. What is a POLST and do I need one? What is an Advance Directive and do I need one? We will have Advance Directive copies on hand and will allow time to complete it, for those that wish to do so.
